Person Centred Care

In the realm of health care, person-centered care is an approach where patients actively engage in their own medical treatment in collaboration with health professionals. At times, family members may also contribute to the development of the patient’s health plan. This approach stands in contrast to traditional treatment models, which often view patients as passive recipients of medical interventions. Traditionally, health professionals may focus more on patient needs than their resources. Unlike the conventional practice of health professionals dictating medical recommendations, person-centered care involves patients and their families in co-creating and agreeing upon medical plans and treatments. This model takes a comprehensive view of the patient’s life situation to establish goals and strategies for both immediate and long-term care.

Person centred care is linked with effective outcomes in health care. Initially developed for elderly patients and those with intellectual disabilities, the approach has been subject to increasing scrutiny and application across various patient groups.

In person-centered care, the patient is seen as an autonomous, capable individual, able to make informed decisions about their health. Autonomy and participation are key values, offering patients the opportunity to be actively involved and take charge of their own health and treatment.

Four key principles underpin person-centered care:

  1. Health care should address the unique needs of the individual and their right to health.
  2. Health institutions should emphasize the abilities of the individual and promote activity.
  3. Health care should be coherent and integrated.
  4. Health professionals should treat patients with dignity, compassion, and respect, adhering to ethical standards.

Person-centered care adopts a holistic view of health care, considering the entire person rather than just focusing on the illness or symptoms. It encompasses the individual’s abilities or resources, desires, overall health and well-being, as well as social and cultural factors.
